If you are a Baylor Scott & White Health Plan (BSWHP) member exploring Spravato (esketamine) for depression, you probably have one big question: will my plan help pay for it? It is a fair thing to ask, and the honest answer is that it depends on the specifics of your plan and on how out-of-network benefits work for you.

We will be upfront about one thing right away, because it matters: NeuroGlow is not in-network with Baylor Scott & White Health Plan. That does not necessarily mean your plan won’t help — many plans include out-of-network benefits, and there are a few ways coverage and savings can still come into play. This guide walks through how Spravato coverage generally works, why your BSWHP plan type matters, what out-of-network means in practice, and how our benefits team can check your specific out-of-network benefits for you.

This article is patient education, not medical advice. Your care decisions should always be made with a qualified clinician such as our medical director, Dr. Aaron Brown, DO.

First, the network question — answered honestly

NeuroGlow is an out-of-network provider for Baylor Scott & White Health Plan. We would rather tell you that plainly than have you find out later. Here is what it can mean for you in practice:

  • Many plans have out-of-network (OON) benefits. Depending on your BSWHP product, your plan may still reimburse a portion of out-of-network care after any out-of-network deductible and coinsurance. Our benefits team can check your specific out-of-network benefits before you commit to anything.
  • Some plans have limited or no OON coverage. Certain HMO and Medicare Advantage designs cover out-of-network care only in emergencies. If that is your plan, we will tell you, and we will talk through your options rather than leave you guessing.
  • A superbill is often available. For eligible patients, we can provide an itemized superbill you can submit to BSWHP for possible out-of-network reimbursement. Our guide on how to submit a superbill walks through that process.

The bottom line: out-of-network is not a dead end, but it is different from in-network, and the details depend on your exact plan. That is precisely what our team can verify for you at no charge.

Baylor Scott & White Health Plan is not just one plan

One of the most useful things to understand up front: BSWHP is a large Texas health plan that offers several different product types. Coverage rules — including how out-of-network benefits work — can differ meaningfully from one product to the next, so knowing which type you have is the first step.

The main BSWHP product types

  • Commercial and employer plans. Coverage you get through your job or purchase directly. These plans set their own medical-necessity and prior authorization rules, and many include out-of-network benefits.
  • ACA marketplace plans. Individual and family plans purchased through the Health Insurance Marketplace, with their own benefit designs. Out-of-network coverage varies and is sometimes limited.
  • Medicare Advantage plans. Government-based plans for Medicare-eligible members. These follow Medicare-related rules, and many cover out-of-network care only in specific circumstances.

Why does this matter for Baylor Scott & White esketamine questions? Because whether your plan reimburses out-of-network care, how Spravato is billed, and what you might pay can all vary by product type. A commercial employer plan and a Medicare Advantage plan under the same BSWHP umbrella can be handled quite differently. You can read more general background on our insurance overview and our Spravato insurance coverage pages.

How Spravato coverage generally works

Spravato is FDA-approved for treatment-resistant depression (used on its own or with an oral antidepressant) and for depressive symptoms in major depressive disorder with acute suicidal ideation or behavior (used with an oral antidepressant). Because it is FDA-approved for these uses, many insurers will consider it for coverage when the clinical criteria are met — including, for some plans, on an out-of-network basis.

Here is what the process typically looks like, keeping in mind that every plan is different:

  • Prior authorization (PA). Most plans require approval before treatment begins. This is a normal, expected step, not a sign your plan will say no.
  • Medical-necessity documentation. Plans commonly want to see your diagnosis and treatment history, often including prior antidepressant trials.
  • REMS-certified setting. Spravato is administered in a certified clinic under monitoring for safety. All three NeuroGlow locations are REMS-certified.
  • Medical vs. pharmacy benefit. Spravato is often billed under the medical benefit rather than pharmacy, though how a specific plan handles this can vary.

So, does Baylor Scott & White cover Spravato? Some plans consider esketamine for its FDA-approved uses, but for care at NeuroGlow that would be under your out-of-network benefits, and that is plan-dependent. Rather than guess, our benefits team can check your specific BSWHP out-of-network benefits and explain what your coverage — and any superbill option — looks like. Our step-by-step Spravato prior authorization page explains the PA process in more detail.

Savings program note for commercial members

This section applies only to eligible, commercially insured patients. If you have a commercial or employer-based BSWHP plan, you may qualify for the manufacturer savings program, which applies to the cost of the medication regardless of network status. Under that program, eligible, commercially insured patients may pay as little as $10 per treatment for the medicine, subject to the program’s terms and limits.

Important caveat: this savings figure does not apply to BSWHP’s Medicare Advantage plans or to any government-funded plan. Patients with Medicare, Medicare Advantage, Medicaid, or other government coverage are not eligible for the manufacturer copay savings program. If your coverage is through a government plan, your out-of-pocket picture will be different, and we will walk you through what applies to you.

How NeuroGlow checks your BSWHP benefits and handles PA

Even though we are out-of-network with BSWHP, we built our intake process to take the insurance guesswork off your plate. Here is how we help:

  • We check your out-of-network benefits at no charge. Give us your BSWHP plan information and our benefits team can look into what your plan offers for out-of-network care and what that could mean for you.
  • We handle prior authorization for you, free of charge. Where a plan requires it, we gather the clinical documentation, submit the PA, and follow up so you do not have to chase paperwork.
  • We can provide a superbill. For eligible patients, we prepare an itemized superbill you can submit to BSWHP for possible out-of-network reimbursement.
  • We keep you informed. You will know what to expect regarding coverage, any required steps, and potential costs before treatment begins — no surprises.

Because we do this across commercial, marketplace, and Medicare Advantage plans, we can tailor the conversation to whichever BSWHP product you have. There is no cost and no obligation to have us check.

Frequently asked questions

Is NeuroGlow in-network with Baylor Scott & White Health Plan?

No. NeuroGlow is out-of-network with BSWHP. That does not automatically rule out coverage — many plans include out-of-network benefits, and for eligible patients we can provide a superbill to submit for possible reimbursement. Our benefits team can check your specific out-of-network benefits at no charge. Call 469-218-4410 or request a free benefits check.

Does Baylor Scott & White Health Plan cover Spravato?

Coverage is plan-dependent. Some plans consider Spravato for its FDA-approved uses, but at NeuroGlow that would fall under your out-of-network benefits, which vary by product type and individual plan. Our benefits team can check your specific BSWHP out-of-network benefits and explain your options.

Can I still get reimbursed if NeuroGlow is out-of-network?

Possibly. If your BSWHP plan includes out-of-network benefits, you may be reimbursed for a portion of care after any out-of-network deductible and coinsurance. For eligible patients, we can provide an itemized superbill to submit to your plan. Some plans, however, have limited or no out-of-network coverage — we will check yours and tell you honestly.

Will I need prior authorization for esketamine?

Most plans require prior authorization before Spravato treatment starts. This is a routine step. Where your plan requires it, NeuroGlow prepares and submits the PA on your behalf at no charge, including the supporting clinical documentation many plans request.

How much will Spravato cost me with BSWHP?

Your out-of-pocket cost depends on your specific plan’s out-of-network benefit design, deductible, and coinsurance. Eligible, commercially insured patients may qualify for manufacturer savings on the medication itself, but that program does not apply to Medicare Advantage or other government plans. We will give you a clear picture once we check your plan.

How do I get started?

The easiest first step is a free Spravato benefits check. Share your BSWHP plan details and we will look into your out-of-network benefits and any superbill option, then handle any prior authorization at no charge. Call 469-218-4410 to begin.
