If you have UnitedHealthcare and your doctor has mentioned Spravato (esketamine), one question probably comes first: will my insurance help pay for it? It is a fair question, and an important one. Here is the short, honest answer, followed by everything you need to understand how the process actually works.

Does UnitedHealthcare cover Spravato?

In most cases, yes. UnitedHealthcare (and its Optum pharmacy arm) generally covers Spravato for its FDA-approved uses, but not automatically. Coverage always depends on your specific plan and requires prior authorization.

Spravato is the only FDA-approved form of esketamine, cleared for treatment-resistant depression (depression that has not improved after other antidepressants) and for depressive symptoms in adults with major depressive disorder who have acute suicidal ideation or behavior. Because it is FDA-approved for these conditions, it is treated very differently by insurers than off-label options.

What that means for you: if you have a qualifying diagnosis and meet your plan’s requirements, UHC will often approve Spravato. The path runs through a prior authorization, and the details vary by plan. That is exactly the part NeuroGlow handles for you.

How UnitedHealthcare Spravato coverage works

Insurance coverage for Spravato involves a few moving pieces. None of them are complicated once you see how they fit together.

Medical benefit vs. pharmacy benefit

Spravato has to be given in a certified clinic and monitored afterward, under a federal safety program called REMS. Because of that, Spravato is commonly billed under your medical benefit (the clinic buys the medicine and bills your plan), rather than picked up at a retail pharmacy.

That said, whether your Spravato falls under the medical benefit or the pharmacy/specialty benefit truly varies by plan. Some UHC and Optum plans route it one way, some the other. This matters because it affects how it is authorized and billed, and it is one of the first things we confirm when we verify your coverage.

Prior authorization

Prior authorization (PA) is required for Spravato. This is simply UnitedHealthcare’s process of reviewing your treatment history and diagnosis before approving the medicine. Optum often manages the esketamine prior authorization on UHC’s behalf.

A PA is not a rejection. It is a documentation step, and a well-prepared submission moves much faster than a rushed one. You can read more about what goes into it on our Spravato prior authorization page.

Step therapy

UnitedHealthcare typically expects “step therapy,” meaning you have tried other treatments first. For Spravato, plans often look for at least two antidepressant medications tried for an adequate period before approving esketamine. This is typical and plan-dependent, not a fixed universal rule, and some states have shorter requirements.

If you have already worked through a couple of antidepressants without lasting relief, you may already meet this expectation. Your provider’s records help tell that story clearly.

Reauthorization

Approvals are not always open-ended. UnitedHealthcare commonly grants Spravato authorizations for a period of time (often up to around twelve months, framed generally), after which your provider documents your progress and requests continued coverage. This is routine for ongoing specialty treatments, and NeuroGlow tracks these dates so your care is not interrupted.

How NeuroGlow gets you approved

This is where a certified clinic makes your life easier. NeuroGlow is REMS-certified with three Dallas–Fort Worth locations in Fort Worth, McKinney, and Flower Mound, and we handle the insurance legwork for you.

Here is what we do at no charge:

  • Verify your specific UHC or Optum benefits. We confirm whether Spravato falls under your medical or pharmacy benefit and what your plan requires.
  • Handle the prior authorization. We prepare and submit the PA, including the clinical documentation your plan wants to see.
  • Manage step therapy and reauthorization. We help document your treatment history and keep your authorization current so care continues smoothly.
  • Explore gap options when needed. If a network issue comes up, we look at solutions like a gap exception.

You do not have to become an insurance expert to get treated. That is our job. Learn more on our Spravato insurance coverage page or our general insurance overview.

UnitedHealthcare and IV ketamine are different

It is easy to lump Spravato and IV ketamine together, but insurers do not. This distinction matters for your wallet.

Spravato is FDA-approved, which is why UnitedHealthcare generally covers it for approved diagnoses. IV (racemic) ketamine, by contrast, is used off-label for depression. It is not FDA-approved for that use, and it is usually self-pay rather than covered by insurance.

So if you are comparing options, know that the coverage picture is not the same. We break down UHC and IV ketamine specifically on our UnitedHealthcare and ketamine treatment page. If insurance coverage is a priority for you, Spravato is typically the more coverage-friendly route.

What UnitedHealthcare Spravato cost can look like

Your out-of-pocket cost depends on your plan’s deductible, copay, and coinsurance, which is why a benefits check is so useful. We cannot promise a specific number without seeing your plan.

There is one savings note worth knowing. For the medicine itself, eligible, commercially insured patients may pay as little as $10 per treatment through the manufacturer’s savings program, and UnitedHealthcare commercial members can qualify. Eligibility rules apply, and this applies to the cost of the medicine.

A quick word on what this is (and is not)

This article is patient education, not medical advice. Whether Spravato is right for you is a conversation for you and a qualified provider like Dr. Aaron Brown, DO, and NeuroGlow’s clinical team. Coverage details are never guaranteed and always depend on your specific plan, so the surest way to get clear answers is to let us verify your benefits directly.

Frequently asked questions

Does UnitedHealthcare cover Spravato?

Generally yes for its FDA-approved uses, such as treatment-resistant depression, but not automatically. Coverage requires prior authorization and depends on your specific UHC or Optum plan. NeuroGlow can verify your benefits at no charge.

Do I need prior authorization for Spravato through UHC?

Yes. Prior authorization is required, and Optum often manages the esketamine prior authorization for UnitedHealthcare. NeuroGlow prepares and submits the PA for you, including the clinical documentation your plan expects.

Will I have to try other antidepressants first?

Usually. UnitedHealthcare typically expects step therapy, often at least two antidepressant medications tried for an adequate period before approving Spravato. This is typical and plan-dependent, and some states have shorter requirements. Your treatment history helps document that you qualify.

Is Spravato billed under my medical or pharmacy benefit?

It varies by plan. Because Spravato is administered in a certified clinic under REMS, it is commonly billed under the medical benefit, but some plans route it through the pharmacy or specialty benefit. We confirm this when we verify your coverage.

How much will Spravato cost me with UnitedHealthcare?

It depends on your plan’s deductible, copay, and coinsurance, so a benefits check gives the clearest picture. For the medicine, eligible, commercially insured patients may pay as little as $10 per treatment through the manufacturer’s savings program, and UHC commercial members can qualify. Eligibility rules apply.

Does UnitedHealthcare cover IV ketamine too?

Usually not. IV (racemic) ketamine is used off-label for depression, is not FDA-approved for that use, and is typically self-pay. Spravato is FDA-approved and generally more coverage-friendly. See our UnitedHealthcare and ketamine treatment page for details.
